I'm sorry to hear you are having trouble with your phone.
Just to clarify, is when making or answering a call that the ear speaker stops working? How does your phone react if you start the test for Proximity switch/Ear proximity?
Considering that the speaker works in Diagnostics my guess is that this isn't related to hardware. Could you try rebooting your phone to safe mode and make a call, is there any change then or are you experiencing the same behaviour?

I see, in that case I would advise that you perform a software repair using Xperia Companion on your phone using a computer. Do note that this will unfortunately reset your phone in the process. Note however, that you can make a backup directly in Xperia Companion using the Backup function.
But I would recommend that you try making a call directly after the software repair without restoring a backup in order to rule out corrupt or damaged data causing this.
