Volume behaving erratically

Started having this weird problem a few weeks ago on MM. Currently on Nougat 32.3.A.0.736.

I've noticed that the "headset plugged in" notification was now gone in Nougat, where it was previously there on MM. Is this a bug or a new thing with Nougat?

For some random reason, the media volume on my phone would just either mute itself, or turn it self to max when listening to music using the Music App or my fav JetAudio Player. Happens mostly when moving about but sometimes it'll just also stop when there is no movement also. At times, the music would just play through the speakers and several times it would bring up the annoying female voiced Google App Voice search.

Another weird thing is that sometimes the volume slider appears and doesn't go away and also the slider doesn't allow you to set your desired volume level. It will sort of rubber band itself to whatever level it wants regardless of how you set it. I've had to unplug the earphones just to solve this issue many times. Restarting the phone too solves nothing.

I've scoured the forums and I've seen many ppl having these same issues and I've tried all the methods that would help solve it but to no avail.

Disabled the Google App.Still happened

Cleared up Smart Connect. Still happened

Booted up in safe mode. Still happened

Factory reset and tried using the headset as antenna for the FM Radio. Still happened

It doesn't appear to be a headphone or audio jack problem as I've tried different headphones and they all seem fine. Perhaps it's the audio jack circuitry being shorted somewhere, and if that's the case, getting it replaced is easy.

So is this a case of hardware failure?
