SD card is not working properly on my Sony Xperia C

Dear Sir,

i have inserted my old Sandisk, 4 GB card in my new sony xperia C model, first it started working fine, but after sometime all the images & songs got disappeared, i want to know why it happened & how it can be rectified.

I've seen this before on other threads, and the problem was a faulty SD card, you could reformat your SD card and check again
